The Challenge
Indonesia's healthcare system faces severe doctor-to-patient ratios in rural areas. Medika Connect wanted to bridge this gap with a reliable, low-latency video consultation product — but needed it to work on 3G connections and integrate with existing clinic EMR systems.
Our Solution
We built adaptive WebRTC streaming that degrades gracefully on poor connections, falling back to audio-only without dropping the session. A GraphQL API unified patient records, prescriptions, and clinic scheduling into a single data graph — reducing EMR integration time from weeks to hours.
The Rural Healthcare Gap in Indonesia
Doctors per capita in rural East Java hover at 1 per 5,000 residents. Medika Connect's mission was to let urban specialists serve rural patients via telehealth — without requiring hospital-grade internet.
WebRTC at 3G: Adaptive Streaming Architecture
We implemented adaptive bitrate WebRTC using simulcast tracks. On weak connections the client automatically switches to a 240p audio-priority stream. Session continuity is preserved through an STUN/TURN relay cluster on AWS.
Digital Prescription & EMR Integration
Doctors issue signed digital prescriptions directly from the consultation screen. We built a GraphQL federation layer that speaks both our native schema and the ICD-10-based schemas used by partner clinics' EMR vendors.
